Patient Resources

Consultations:

The first step to becoming a patient at Lakeside Cosmetic Center is to schedule your consultation. A consultation involves a medical health questionnaire, office forms, photos and a meeting with one of our providers to discuss your concerns and goals for treatment or surgery.

During the consultation, a comprehensive treatment plan is customized specifically for you and your needs. We strive to answer all of your questions and make you feel comfortable with your treatment plan and options.

Our Patient Care Coordinators will be able to further explain our consultation process, make recommendations on the appropriate provider for you and associated fees.

Cancellation Policy:

We require 48 hours notice for cancellations or requests to change your appointment. No shows, late arrivals, and last minute cancellations prevent another patient from being cared for. Lakeside Cosmetic Center reserves the right to reschedule late arrivals of more than 15 minutes.

If you fail to give 48 hours notice to reschedule, cancel, or “no-show” to your appointment, a $50 fee will be billed to your account. This fee must be paid prior to your next appointment. Thank you for your understanding and cooperation as we strive to best serve the needs of all of our patients.

Financial Policy:

We accept American Express, Discover, MasterCard, Visa, Care Credit, Cash, or Check as methods of payment. There will be a $35 return check fee for all returned checks.

 

Payment is due in full at time of service for all non-surgical appointments and procedures. All treatment plan pricing will be reviewed prior to performing the procedures, to make sure you are comfortable with the cost. Fees for surgical costs are paid in full prior to surgery.

We proudly participate in Alle by Brilliant Distinctions for all Allergan products (including: Botox, Juvederm and CoolSculpting) and Aspire rewards for all Galderma products (including Dysport and Restylane).
